If you are a contractor or subcontractor, you might need to fill out a Statutory Declaration of Progress Payment Distribution by Contractor (Form 9A) or Subcontractor (Form 9B), which has been made available to contractors and subcontractors by the Canadian Construction Documents Committee (CCDC) for progress payments and holdback releases.
Both the contractor and the subcontractor who is requesting progress payment for the release of their progress or holdback payments must use this form. This form or document guarantees that the correct payment is paid throughout the whole execution of the project. It is important to state that the CCDC document must be witnessed by persons authorized to administer oaths. The CCDC document comes in two flavors, depending on whether you are a contractor or a subcontractor.
